The information within this section may seem confusing so to summarise:

Cremation of a death not subject to a procurator fiscal report

Once the death occurs a Form 11 is issued. The registrar keeps this document and issues a Form 14, a Form BD8 and any number of extracts the family require. The family arrange the funeral with ourselves and complete a Form A. We will arrange with the doctors to sign Forms B and C. We will take Forms 14, A, B and C to the crematorium.

Cremation of a death subject to a procurator fiscal report

Once the death occurs and is cleared by the procurator fiscal a Form 11 is issued. Prior to going to register the death, the family should take the Form 11 to the procurator fiscal's office to obtain a Form E1. Then they should proceed to the registrar. The registrar keeps the Form 11 and issues a Form 14, a Form BD8 and any number of extracts the family require. The family arrange the funeral with ourselves and complete a Form A. We will take Forms 14, A, and E1 to the crematorium.

Burial of a death either subject or not subject to a procurator fiscal report

Once the death occurs (and if necessary cleared by the procurator fiscal) a Form 11 is issued. The registrar keeps this document and issues a Form 14, a Form BD8 and any number of extracts the family require. The family arrange the funeral with ourselves and, if available, give us the Title Deeds. We will take Forms 14 and the Title Deeds to the cemetery.

Certificate of Delivery of a baby of less than 24 weeks gestation - This is completed and signed by a registered midwife or a medical practitioner. It will be handed by the family to ourselves who will give it to the cemetery or cremation authority. It will be retained by the authority.

Certificate of Stillbirth (Form 6) - This is signed by a doctor and is required for registration of a stillbirth. It will be retained by the registrar.

Certificate of Registration of Stillbirth (Form 8) - This is issued by the registrar of the district at the time of registration of stillbirth. It will be handed by the family to ouselves who will give it to the burial or cremation authority. It will be retained by the authority.

Medical Cause of Death Certificate (Form 11) - This is signed by a doctor and is required for registration of death. It will be retained by the registrar.

Certificate of Registration of Death (Form 14) - This is issued by the registrar of the district at the time of registration of death. It will be handed by the family to ourselves who will give it to the burial or cremation authority. It will be retained by the authority.

Extract of an Entry in the Register of Deaths - Commonly known as a 'copy of the death certificate', this is issued by the registrar of the district at the time of registration of death. There is a modest fee for this and is required for insurance, banking, pension or other purposes. This is the only official document which will be left with the family after the funeral has taken place.

Registration or Notification of Death Form (Form BD8) - This is issued by the registrar of the district at the time of registration of death. It is for use in adjusting Social Security Benefits.

Application for Cremation Certificate (Form A) - This is required for all cremations and is completed and signed by a responsible person representing the family (applicant) and ourselves. We will give it to the cremation authority. It will be retained by the authority.

Certificate of Medical Attendant (Form B) - This is required for cremation and is completed and signed by a doctor who has attended the deceased before death and who has seen and identified the deceased after death. We will give it to the cremation authority. It will be retained by the authority. In many cases we will liaise with the doctor regarding this form.

Confirmatory Medical Certificate (Form C) - This is required for cremation and is completed and signed by a doctor of not less than five years standing who has seen the deceased after death. We will give it to the cremation authority. It will be retained by the authority. In many cases we will liaise with the doctor regarding this form.

Procurator Fiscal Certificate for Cremation (Form E1) - This is required for cremation where there is procurator fiscal involvement in the death and supercedes Forms B & C. We will give it to the cremation authority. It will be retained by the authority.

Stillborn Child Certificate for Cremation - This is required for cremation. It is completed by a registered medical practitioner who was present at birth and is needed instead of Forms B & C. We will give it to the cremation authority. It will be retained by the authority.

Title Deed - This certificate is required for burial and is issued at the time of purchase of a grave in a cemetery. If this is lost or not produced then an indemnity is required to be signed at the cemetery office. In some cases this is only permitted by the eldest offspring of the eldest offspring of the title holder etc. Title deeds, if produced for burial are returned to the family after the funeral.
